Optional Student Accident Insurance for the 2021-2022 School Year

Parents are encouraged to purchase optional Student Accident Insurance.
The school district is happy to provide parents with the opportunity to purchase accident insurance to cover their students during the school day as well as 24-hour coverage.

The application form (in both English and Spanish) is attached. Parents are given the option of purchasing dental coverage as well. Benefits are coordinated with any other health insurance the student may have and are paid according to a schedule depending on the level purchased by the parent.

The claim form is also attached should the parent purchase a policy and their student have an accident.
